• for each rate object, int_combined_raw contains the hospital MRF rates, payer MRF rates, Komodo allowables + charges, and first party rates.

Note that in Trino, each rate column has a corresponding id column (omitted in data dictionary below). For example, e.g. payer_derived_rate and payer_derived_rate_id; this id column is used to trace the rate back to the original source
