  • All Hospitals.
  • Ambulatory Surgery Centers
  • Laboratories
  • Select Physician Groups

• Codes:

  • All Inpatient Encounters
  • Top 5000 Institutional Level 1 HCPCS (by revenue)
  • Drug Primary Rates: Any HCPCS codes that both (1) have a Medicare ASP OR (2) have OPPS status indicator G or K
